HEAR FROM OUR PATIENTS
"As an active person in my late 50s, a sudden back injury was a major setback. Danielle and the Formula team created a rehab program before and after my fusion surgery that helped me avoid missing a ski or golf season.
I now trust them as my ongoing partners for mobility, strength, and performance."
Tom H.
“I’ve trusted this clinic for six years for everything from tendonitis and arthritis to post-surgical recovery. Dry needling and shockwave therapy have delivered real results every time.
My kids have come here for sports injuries, and I’ve referred countless coworkers because the care is always genuine, attentive, and personal. Patients are treated like family.
Jack W.
"I have been seen by Dani at various times for different sport-related (tennis, golf, weight lifting) injuries and have always achieved great short-term and long-term results. I most highly recommend this practice. "

“I first came to Danielle after years of struggling with chronic hamstring tendinosis that kept me from running. Her expertise and thoughtful approach got me back to running — and eventually helped me qualify for the Boston Marathon.
She remains my first call for any physical therapy issue, and I recommend her without hesitation.”

FREQUENTLY ASKED QUESTIONS
Your questions, answered.
Everything you need to know before your visit — from insurance and scheduling to what to expect during treatment.
Insurance coverage can vary depending on the therapist and your specific plan. Our team works with a variety of insurance providers, and in some cases we can also submit out-of-network claims.
Because coverage differs between plans, we recommend calling the office so we can help verify your benefits and discuss the best options for your care.
Most appointments are approximately 60 minutes, allowing time for thorough assessment, hands-on treatment, and guided exercise. Once your initial evaluation is completed, session length can occasionally be adjusted slightly to accommodate your treatment plan or schedule.
Comfortable clothing that allows access to the area being treated is recommended.
• Shoulder, neck, or elbow: a t-shirt or tank top
• Hip or knee: shorts or athletic shorts
• Ankle or lower leg: loose-fitting pants that can be rolled up
This allows your therapist to properly evaluate movement and perform hands-on treatment.
In most cases, dry needling and shockwave therapy are not covered by insurance plans. We offer treatment packages for each service, as well as combined therapy options. Please contact the clinic for current pricing and package details.
In Connecticut, physical therapists have direct access, which means you can begin treatment without a physician referral.
However, some insurance plans may require a referral or prior authorization after a certain number of visits.
Because coverage can vary, we recommend calling the office so we can guide you based on your specific plan.
Your first appointment includes a comprehensive evaluation, where your therapist will assess your movement, discuss your goals, and develop a personalized treatment plan.
We work with individuals recovering from injury, managing chronic pain, or looking to return to sport and activity safely. Common conditions we treat include:
- Running and sports injuries
- Shoulder, hip, knee, and ankle pain
- Tendon injuries and overuse conditions
- Post-surgical rehabilitation
- Neck and back pain
- Balance and vestibular conditions, such as vertigo
- Movement limitations and performance plateaus- Pelvic floor therapy for men and women.
If you’re unsure whether physical therapy is right for your condition, our team is happy to help guide you.
